   -Team Name
   -Division [open, mixed, or women's]
   -Contact Name & Email Address
   -Previous years played at Cal States
   -2006 Regionals placing
3. Check $275 per team to:

4. Bids will be accepted on a rolling basis

All 2006 Cal States semi-finalists are guaranteed a bid.
