Becoming Certified

First Steps:

1) Decide which Progression Of Classes you wish to take and review the
Fall Calendar or Spring Calendar.

2) Review the
Fall or Spring Information Sheets as well as the Fall or Spring Syllabus.
2a) Access additional information about
UCSC's Biology Classes.

3) Print and complete an
Application and the Equipment List.

4) Print and complete the
Medical Forms. Obtain your doctor's signature.

5) Mail the application and medical forms to: UC Dive Safety Program, Center for Ocean Health, Rm 205, LML, 100 Shaffer Road, Santa Cruz, CA 95060

6) Review
Equipment Hints and prepare your equipment.
