Information on Payroll Deduction for
Faculty and Staff Wellness Cards
Payroll Deductions are available to Faculty and Staff. The sign-up periosd is from late May through early Novemeber. Payroll Deduction are not a monthly fee, it is the cost of the card for the Academic year broken into smaller payment using the Payroll Deduction. We are happy to offer this service the Staff and Faculty of UCSC, but please be sure that it is the right option for you.
If you decide to your card any longer, you will not necessarily be able to discontinue your deduction that month. There will be a $30.00 cancellation fee for early termination of your card in addition to the full cost of the quarters for which the card was usable.
Wellness Cards and Payroll Deductions must be renewed each year. To renew you must submit a completed Payroll Deduction Form and a signed and dated Facility Waiver Form. Wellness Cards must be renewed or issued in person. The same card will be reused for renewals.
To be eligible for payroll deductions you must meet the following criteria:
*You work 12 months a year.
*You work at least 50% time.
*Your current appointment ends after June 30, 2008.
*You don't want to pay the entire cost of the Wellness Card up front.
*You don’t want to come to OPERS to update your card every quarter.
*You want your recreation card for at least 3 quarters within the current academic year. (Fall to Spring).
*You want to have a great price for the 4 quarters within the current academic year (Summer - Spring).
*It is during the sign-up period.
