HR Forms Library
Day-to-day university business requires the collection
and distribution of information in a standardized way. To accomplish
this, forms are to be used in conjunction with local procedures
to implement policies, collective bargaining agreements (contracts)
and programs for UCSC employees. Links to appropriate forms are
contained within a particular policy or program web page. As
an alternative, the Staff HR Forms Library provides direct links
to forms grouped either by topic or by title.
Available Document Types
- Forms (in .doc or .pdf
format) are designed to collect employee information and
any necessary signatures required to complete an action as
specified by policy. (See Tips
for Using Adobe Forms)
- Sample notices (in .doc format) provide templates with
specific language and disclaimers required by policy and contracts,
allowing departments and supervisors to customize notices for
individual employees.
Both types may be saved to the user’s workstation as a
file or printed out prior to entering data. Users should check
periodically for updated forms, especially following a change
in the governing policy or program. These changes are announced
on the main Staff HR Home Page.
Navigation
The menu bar at the left provides three ways to locate information
in the Staff HR Forms Library.
- The Forms Grouped by Topic menu contains links to forms grouped by
topic. Many forms will be listed under more than one topic.
- The Forms Listed by Form Title menu contains links to forms grouped alphabetically
by form title.
- Use the Search Field and "go" button located
at the top left of the page to perform a text search across
all forms. This search is limited to information contained
in the Staff HR Forms Library.
