Medical Services: Medical Records
- We maintain medical records charts for all patients seen at the Health Center within the last 10 years and in some cases longer (i.e., when special regulations apply). We handle about 500 charts daily.
- We provide copies of medical records, partial or complete, when needed for care. A written release from the patient is required to provide time to review, copy and prepare the record(s) for mailing, faxing or pick-up. You can download the request form - mail, fax or deliver in person the completed form to our office. Please allow approximately five working days to process your request. Requests for records stored off site can take up to ten or more working days to process. When it is urgent, we will fax records directly to the medical clinician you have designated on the form.
- We process subpoena requests and insurance inquires.
- We offer a lab results message center. Generally, when results are within normal limits, the Health Center does not routinely contact patients. If test results return abnormal or unexpected, the Health Center will make every effort to contact the patient with this information and provide follow-up advice and instructions.
Our lab result phone line allows you to call and request a clinician call you with the results of your most recent tests. Call 459.2591 and follow the instructions. We will try to return your call before the end of the following working day.
Policy:
- Medical records will not be released without written permission from the patient.
- To ensure privacy and the protection of records, you will be required to show identification when requesting medical records. Please be prepared to provide a picture ID and your UCSC student ID.
Fees:
- A fee of 10 cents per page is charged for medical records for personal review or use.
- Fees are waived when records are sent directly to a medical clinician for treatment or transfer or care.
- A regulated fee is charged for medical records prepared in response to a subpoena.
