Hepatitis "B" Requirement

State law mandates all students under 19 years old be immunized against Hepatitis "B." If you have not received this series of three injections, we suggest you begin now. We must receive documentation that at least the first immunization has been given by the first day of class instruction. By the third quarter, we must have documentation that you received all three immunizations. A Hepatitis "B" form has been sent to all incoming students, however you can also download this form from the bottom of this page. Immunization may be waived on principle of personal beliefs, but you must still complete and return the form by the required date.

If you have completed or begun the Hepatitis "B" Series
We will accept a copy of the doctor or clinic's record, or your health care provider can sign the Hepatitis "B" form. You may also send a copy of your Immunization Record. Please do not send the original.

Where do I get these shots & what do they cost?
Some insurance plans, such as the Undergraduate Health Insurance Plan (UHIP) cover the cost of these immunizations as preventative medicine. Please check your insurance plan. Your usual health care provider should be able to either administer or refer you elsewhere for these shots. County and city health departments may provide this service for free. The UCSC Student Health Center also provides these immunizations for approximately $70, per immunization, which may be reimbursable by your insurance. Even if you began the series elsewhere, we can provide your follow-up shots.

How do I get my Hepatitis "B" shots at UCSC?
We will be open for drop-in Hepatitis "B" during move-in weekend. At those times Meningitis immunizations will also be available. After school begins, follow-up immunizations will be scheduled by appointment through our Allergy, Immunization and Travel Clinic at 459.2500.

What about the paperwork?
The Student Health Center receives and reviews documentation regarding compliance with the Hepatitis "B" requirement. We will update your records as you provide us with new information. The Registrar's Office is notified when students are not in compliance with this regulation, and their ability to attend classes may be jeopardized.
