UCSC EAP HEALTH CLEARANCE INSTRUCTIONS
READ THE FOLLOWING INSTRUCTIONS CAREFULLY
All students require a Health Clearance to study abroad.
You are responsible for completing this process by the EAP deadline.
The EAP clearance process is designed to safeguard your health abroad. It is in your personal interest to be open and candid about your health issues.
Students must inform the University-wide Office of EAP (UOEAP) of any recent medical or special needs or changes in their health occurring before the start of the EAP program. UOEAP will use this information to help you determine if adequate medical or other resources are available in your program abroad.
Failure to provide complete and accurate information may be grounds for non-participation in or dismissal from EAP.

STEPS TO COMPLETING THE HEALTH CLEARANCE PROCESS
- Most students may complete their Health Clearance process on-line.
The fee for this is $20 and will be charged to your University account.
- Other students may require appointments for travel education, tests, and/or a physical exam at the Student Health Center or with their physician, psychiatrist or specialist. There are various fees associated with the EAP Clearance process.

- All students are encouraged to research their destination country at :
- Create a Secure Messaging account if you are entering the UCSC Student Communicator web site for the first time. Go to https://studenthealth.ucsc.edu/ (Note you must have an active UCSC email account to access this secure on-line resource.)
- Select the Sign-up for an account link and follow the directions.
- An access key will be mailed to your UCSC email address. This access key is required to confirm your identity and complete your account setup.
- First-time users must read and acknowledge the Student Health Center Notice of Privacy Practices.
- Log in and select Messages, then select New Messages, and then select Education Abroad Program Clearance to complete the Confidential Health History.
- Once completed, print out and make several copies of your Confidential Health History form.
- Take several copies abroad with you.
- Do not send it to UOEAP or give it to your EAP adviser.
- You will have the only copies of this health information at your program site abroad in case you need medical attention.
- Within 14 days after on-line submission of your Confidential Health History, you will receive an e-mail message from the UCSC Student Health Center alerting you that you have a secure message waiting.
- You will be instructed to pick up your signed and completed EAP Health Clearance form at the Student Health Center. You will be directed to come to Student Health Center reception desk to pick up your EAP Health Clearance form. You must show your student ID at this time.
- Mail the white and pink copies to UOEAP(University- wide Office of EAP) at:
- UOEAP must receive the form no later than 60 days before your departure.
- Do not mail your copies of the Confidential Health History form.
- If you are seeing a specialist for any new or on-going medical or psychological condition each specialist must approve and sign the health clearance form and include an address and phone number. The Student Health Center may give only conditional clearance pending specialist approval and clearance. You will be e-mailed instructions if you require additional appointments or specialist approval before clearance is given.
- You must have approval from each specialist and the Student Health Center provider before being cleared for EAP participation.
- After you obtain your specialist’s approval and signature make an appointment at the Student Health Center for final EAP clearance. Call 831-459-2500 to set up an appointment.
- Bring all 3 copies of the EAP Health Clearance Form to the Student Health Center appointment.

- Complete a Quiz
- Attend a Travel Consult
- Go to the Country Grid and choose your destination country.
- Select “download quiz” and print out the quiz for your country.
- Go to the website www.tripprep.com. Create a user name and password, following the directions.
- Select your country from “Destination information”. Use this information to complete the travelhealth quiz.
- Review of your medical history with suggestions for maintaining your health abroad.
- Discussion of specific travel health risks and recommendations.
- Review of immunization history with discussion of required and recommended vaccines.
- Providing prescriptions for anti-malarial medication and antibiotic for travelers’ diarrhea.
- Signing of the Travel Medicine (Clinic) Certification form (if appropriate).
- Schedule an EAP Travel Health Consult in the Allergy, Immunization, and Travel Clinic (AIT) at the UCSC Student Health Center by calling 831-459-2500.
- Print out a copy of the Travel Medicine (Clinic) Certification form:

- (Note: Bring your immunization records with you even if you submitted this information to the Student Health Center in the past. Do not assume that we have these records. You may need to check with your parents, family physician or high school for your immunization records.)
- UOEAP must receive this form by stipulated deadlines, but no later than 1 month before departure. Send the completed original copy of Travel Medicine (Clinic) Certificationform to:

- You may require additional appointments to complete your travel immunizations. Some immunizations require a series of shots to provide adequate immunity; it may take several months to complete the series.
- It depends. EAP recommends that you are up to date on your Tetanus, MMR, Polio, & Hepatitis B vaccines. These are routine childhood vaccines so most of you will be up to date, but some may need boosters.
- A few countries require proof of a Yellow Fever vaccine before entry. Some host universities require the Meningitis vaccine.
- Depending on your destination, other specific vaccine recommendations will be strongly advised, but are not required for participation.

All costs are approximate and subject to change. Please check with your insurance carrier for coverage.
- USHIP pays for up to $250.00 per academic year under the Wellness benefit. (deductible does not apply) Services that may be included under this benefit are lab tests, immunizations & group meetings. Once the Wellness benefit is exhausted, the student is responsible for 100% of the costs.
- Certain prescriptions will be covered under the USHIP pharmacy benefit through the end of your USHIP plan. Any remaining medications can be sold to you at regular fees. The SHC pharmacy also sells many over the counter travel products including insect repellent.

- That depends on the specific country requirements and recommended immunizations. Some destinations require labs, physicals, and other procedures that will take multiple visits. Also, many immunizations are given in series, requiring advance planning and several visits.
- Also, some new or on-going medical or psychological conditions require a specialist’s approval. This may require 1 or more appointments.
- To make an appointment at the SHC call 459-2500 or go to the appointment desk located in the Student Health Center.
- It is the student’s responsibility to ensure that s/he has a sufficient supply of medication for the duration of the program or to verify that the medication is locally available. AIG Assist (1-800-626-2427 in the USA or Canada) can provide information to students on whether a specific prescription medication is legal in another country.
- Custom regulations regarding medications, both prescription and over-the-counter vary by country. Contact the consulate for instructions.
- Letters from the prescribing physician authorizing use of medications may be required for entry.
- Remember, insurance coverage may have restrictions on how much medication can be purchased at one time. Contact your insurance carrier for details.

Step 1 : Completing the on-line Confidential Health History Form (mandatory)
All UCSC EAP students, regardless of the destination country, must complete the Confidential Health History on-line.
Due to specific time constraints, the Confidential Health History must be submitted by stipulated deadlines but no later than 60 days before departure.
Get your complete immunization records (including month, day and year). You will need these to complete this form.
Getting Started:
After being submitted, your Confidential Health History will be reviewed by a health professional at the UCSC Student Health Center. Within 14 days you should receive a secure message informing you of the next steps for your clearance process.

Step 2: Obtaining your EAP Health Clearance
Certain medical or psychological conditions, host countries or universities require in person appointments for evaluation &/or testing before the EAP Health Clearance can be completed. Some countries require completion of the EAP Health Clearance process at the UCSC Student Health Center not your private physician. Several countries require additional forms, letters, and/or documents that off-campus providers may not be aware of.

All UCSC EAP students, regardless of the destination country, must obtain the EAP Health Clearance form. This required document states that at the time of his /or her evaluation the student is considered to have no unstable or unsafe medical or psychiatric issues that would preclude participation in EAP.

Note: The UCSC Student Health Center will not mail your clearance form.
Some UCSC EAP students will require in-person appointments with a health care professional at the Student Health Center or with their physician, psychiatrist and / or specialists before health clearance is given. Note: If you cannot obtain your specialist’s approval and signature you will not be cleared to participate in EAP.
Some countries require additional Travel Medicine certification.

Step 3: Obtaining A Travel Medicine (Clinic) Certification form
Some countries pose more health risk than others. To help students plan a safe and healthy EAP experience, the UCSC Student Health Center or UOEAP (University- wide Office of EAP) may require specialized travel health education. After review of the travel health information, completion of a travel quiz , and a travel consult appointment at Student Health Center’s Allergy Immunization and Travel Clinic (AIT) you will be cleared for travel if appropriate and given the Travel Medicine (Clinic) Certification form.
