Welcome to Student Health Services at UC Santa Cruz!

New & Returning Student Health Information

There are 4 steps you must take before September 17, 2009 to ensure compliance with State regulations and University requirements.

Summer Mailer sent to all incoming undergraduate and graduate students

 

1. INSURANCE (required)

Accept or Waive the Undergraduate Student Health Insurance Plan (USHIP) or Graduate Student Health Insurance Plan (GSHIP) offered by UCSC (all students).

2. HEP B IMMUNIZATION (required)

Complete the online Hepatitis B Statement (REQUIRED for all new students under 19 years of age on September 19, 2009).

3. PERSONAL HEALTH HISTORY (optional)

Complete the online Personal Health History Form (all new students).

4. MEDICAL RECORDS (optional)

Obtain your medical records if you are being followed for a medical condition.

1. INSURANCE (required)

All undergraduate students and all full-time graduate students are automatically enrolled in the university-sponsored Undergraduate Student Health Insurance Plan (USHIP) or Graduate Student Health Insurance Plan (GSHIP) unless they choose to submit an online insurance waiver with proof of comparable coverage.

If you have comparable health insurance coverage and are sure that you want to waive the USHIP or GSHIP plan, follow these instructions:
  1. Go to the UCSC student portal at my.ucsc.edu.
  2. Sign in to the student portal using your User ID and Password.
  3. Select the Student Health Insurance On-line Waiver (FALL QTR). link located in the “Academic News” section. You will be redirected to the website where you can begin the secure waiver process.
  4. Follow the instructions on the waiver web site to complete and submit your health insurance waiver.

If you do not have access to a computer, or for waiver assistance, please contact the Student Insurance office at 831-459-2389 or email insure@ucsc.edu.

Deadline to waive is September 17, 2009

2. HEP B IMMUNIZATION

(REQUIRED for all new students under 19 years of age on September 19, 2009)
  1. Go to the UCSC Student Health Communicator web site at: https://studenthealth.ucsc.edu/
  2. If you are logging into the Student Health Communicator web site for the first time, you will need to create an account. Select the “Sign up for an account” link and follow the instructions.

    NOTE: You must have an active UCSC email account to access this secure on-line resource. An access key will be mailed to your UCSC email address. This access key is required to confirm your identity and complete your account setup. First time users will also need to read & acknowledge the SHC Notice of Privacy Practices. Please follow the instructions on the Home page.
  3. Log in and select “Messages”, then select “New Message” and then choose the “Hepatitis B Status Statement” to complete and submit this secure online form.

3. PERSONAL HEALTH HISTORY (optional)

  1. Go to the UCSC Student Health Communicator web site at: https://studenthealth.ucsc.edu/
  2. If you are logging into the Student Health Communicator web site for the first time, you will need to create an account. Select the “Sign up for an account” link and follow the instructions.

  3. Log in and select “Forms”, then choose “Personal Health History” and “Immunizations” to complete and submit these secure online forms.

4. MEDICAL RECORDS (optional)

Obtain your medical records if you are being followed for a medical condition.

If you or your doctor want the UCSC Student Health Center to provide care or monitor your medications or lab work, please ask your primary care provider to compose a summary of your medical treatment plan. Please DO NOT send your entire medical record.

Records can be faxed to our Medical Records department at 831-459-3546 or mailed to UC Santa Cruz / Student Health Services at return address above.
