Financial Aid Forms

UCSC Financial Aid and Scholarship Office forms are saved in pdf file format and require the free Adobe Acrobat Reader in order to view and print.

Submit verification forms only if requested.  Some UCSC aid applicants may be asked to print, complete, and submit certain forms to verify their Free Application for Federal Student Aid (FAFSA) application data. To determine which documents you are required to submit, please refer to your "To Do List" at MyUCSC or contact our office.

Submit forms by postal mail or in person only. To help safeguard your personal information, we do not accept faxed or e-mailed documents. Mail forms via U.S. mail or bring them into our office. A secure drop-box is also available 24/7 outside the Hahn Student Services Building (near the loading dock).

Keep a copy of all documents you submit.
