Federal Quality Assurance Program

Mission
Quality Assurance helps schools Attain, Sustain, and Advance exceptional student aid delivery and service excellence.

Vision
To broaden the Federal Student Aid Quality Process to serve all participating Title IV institutions by providing tools that promote better service to students, compliance, and continuous improvement in program delivery.

UCSC Process
About 300 students are randomly selected every two years to participate in this program. Our goal is to determine how much, if any, of the data which is currently being collected from students on their financial aid applications (FAFSA), should be verified for accuracy.To measure this, selected students are notified via email and are required to submit documents to verify their FAFSA data.

Participation is mandatory. Selected students must submit required forms.

Note: Fall aid will be held until forms are received and reviewed.

Form Deadline: June 30, 2008

What You Must Do if Selected
1. Determine whether you meet the federal criteria for "dependent" or "independent" status
2. If you are dependent, download the 2008-09 Dependent Verification Worksheet or
3. If you are independent, download the 2008-09 Independent Verification Workheet
4. Complete then print the worksheet and attach a copy of your 2007 federal tax return. Be sure it is signed and include all schedules and attachments.
5. If you are dependent, also attach a copy of your parent 2007 federal tax return. Be sure it is signed and include all schedules and attachments.
6. Submit the completed worksheet with attached tax return copies to the UCSC Financial Aid Office.

Notes:
If you previously sent us a copy of your and/or your parent(s)’ 2007 Federal tax return, DO NOT SEND another copy. In this case, you have completed the verification requirement and no further action is necessary.

Your 2008-09 awards may change depending on the accuracy of the FAFSA information when compared to the information on the Federal Verification Worksheet and tax return. If your aid changes, you will be notified by e-mail.
